Documents

  1. Driver's License/DMV ID or Govt ID ( example Passport) of Taxpayer.
  2. Social Security cards for everyone on the return.
  3. Birth Certificates for all Children on the return.
  4. Health Insurance proof (Form 1095-A if your insurance premium was subsidized by Affordable Health Care).
  5. IF YOU ARE A NEW CLIENT, WE WOULD PREFER TO HAVE ACCESS TO A COPY OF YOUR LAST TAX RETURN FILED.
  6. INCOME  STATEMENTS AND OTHER DOCUMENTS RECEIVED THAT ARE LABELED FOR INCOME TAX PURPOSES.

If you have children in college, or school age children please furnish their school records such as a verification of address document, and any 1098-T document from the school website or financial aid office that they may have from college classes tuition expenses?
